A physical therapy superbill is a comprehensive healthcare invoice containing patient and provider details, therapy session dates, standardized procedure codes, diagnostic codes, service quantities, total charges, payment information, and a provider's confirmation signature. It facilitates accurate billing and communication with insurance for reimbursement.
Clients can submit it to their health insurance for potential reimbursement, depending on the insurer's discretion.
Virginia allows the evaluation and treatment from a licensed Doctor of Physical Therapy without needing a referral from a doctor of medicine, osteopathy, chiropractic, podiatry, or dental surgery, a licensed advanced practice registered nurse, or a licensed physician assistant under the supervision of a licensed physician.
However, it is important to note that if you are currently receiving care from any of the healthcare professionals mentioned above for the same symptoms for which you are seeking physical therapy services, a referral would be required at the time of the physical therapy evaluation.

Please fill out intake forms that you were sent when you made an appointment prior to your first visit. If you didn't receive an email with the forms, please contact us and we will send it to you as soon as possible.
Please bring flexible clothing (shorts/shirt) so that relevant anatomy can be examined.
Please bring any and all medical documents, list of medications or OTC supplements, imaging/reports, detailed surgical or procedure notes, or anything else that may be related to your injury or condition.
If under 18 years of age, please have your parent or guardian briefly attend in your first visit.
In a typical session, we will ask a detailed history of your symptoms, review what the past few weeks have been like, assess any changes from the last visit , and ask any other relevant questions to better understand how you are doing. We will ask how you tolerated the last visit and make changes to your program based on your reports.
We will start with several hands-on manual therapy techniques designed to elicit a specific movement pattern or decrease pain.
Lastly, we will perform corrective exercises designed individually for you. Theses are purposeful movements designed with your intent to improve your quality of life.
Yes, we charge a 100% cancellation fee for no-shows, reschedules, and cancellations within 24 hours of your appointment, excludes packages.
